Office Financial Policies
Thank you for choosing us as your dental care provider. We are committed to the success of your dental treatment. Please understand that payment of your bill is considered part of your treatment, and the following is a statement of our financial policy which we require that you read and sign prior to any treatment. If you have any questions or concerns, please do not hesitate to ask our business office staff.
PAYMENT POLICY
Payment is due in full at time of service unless previous arrangements have been made, including but not limited to copays, deductibles, previous balances, or other fees. We accept Cash, Check, Debit and Visa, Mastercard, American Express and Discover Credit Cards. Payment plans can be arranged as needed.
DENTAL INSURANCE
As a courtesy, we will gladly file claims and accept assignment of dental benefits provided you agree to the following:
Complete insurance information must be provided prior to or at the first appointment for verification. Payment is at time of service for treatment completed without verified insurance.
Estimated co-pays and deductibles are expected at time of service.
All charges not paid by your insurance company are your responsibility, regardless of the reason for nonpayment. An insurance policy is a contract between you, your employer, and your insurance company. Some services may not be a covered benefit with your insurance. It is your responsibility to know your policy.
APPOINTMENT POLICY
We ask you to give us at least a 24-hour notice if you are unable to keep your scheduled appointment. We understand that emergencies and other problems do arise, and we will do our best to accommodate in these situations. Please review the following so that we can best serve you:
Broken appointment: this occurs when an appointment is missed or rescheduled with less than 24-hours notice. A fee of $25 can be charged per instance. We reserve the right to terminate professional treatment of any patient when scheduled appointments are not kept.
Late arrivals: If you are more than 15 minutes late for your appointment, we will do our best to accommodate. If you anticipate that you will be more than 15 minutes late, please call us immediately. If we must reschedule, a broken appointment fee may be charged based on the situation.
